Cares Community Health uses the Patient Centered Medical Home model to provide primary medical care to our patients. Our interdisciplinary team, comprised of the medical provider, nurse, nutritionist and other specialists, works with patients to manage their health, injuries, illnesses and chronic diseases. The self-management model allows patients to be in charge of setting their goals with the support of a professional health care team. Primary care services include preventive care, routine checkups, immunizations and management of acute and chronic illnesses. Our specialized chronic disease program teams focus on asthma, diabetes, hypertension, cardiovascular disease, depression and HIV/AIDS.
Cares Community Health offers preventive care, routine checkups and dental services including: dental exams and cleanings, dental x-rays, oral health education, fillings, root canals, extractions, crowns, bridges, partial and full dentures.
Mental health services include: outpatient counseling for individuals, couples and families; psychiatric evaluation and ongoing medication management; and culturally competent behavioral health treatment for non-English speaking patients. Services for patients include psycho-social assessment, crisis intervention, domestic violence screening and intervention, advocacy and transportation.
Cares Community Health substance abuse treatment services include: screening, assessment, intensive and regular outpatient treatment for adults and youth, group and individual treatment, detoxification services by referral, involuntary commitment services, and co-occurring treatment services for people who have mental health issues in addition to substance addictions.
Case managers connect patients to appropriate community resources and assist them in navigating the health care and insurance systems
Nutrition counseling is available to all children and adults who receive primary medical care at Cares Community Health. Our nutritionists have expertise in eating disorders, weight management, diabetes, hypertension and HIV/AIDS.
Cares Community Health offers an onsite pharmacy. We are able to purchase prescription medications at a discount through special funding from the federal government. We are pleased to offer a laboratory that performs a wide range of testing and services for our patients, including phlebotomy, hematology, chemistry, immunochemistry, serology and urinalysis.
Cares Community Health is a Federally Qualified Health Center – Look Alike. No one will be refused service if they are unable to pay.
